In the lively debate over the protocols for corneal collagen cross-linking, epithelium-on techniques have a special place. A transepithelial treatment has advantages in terms of noninvasiveness and safety, but the barrier effect of the epithelium, attenuating riboflavin penetration, UV light absorption and oxygen diffusion, seemed difficult to overcome.
