While there are several ways to remove an IOL, Loh’s preferred method has four steps: separating the anterior capsule from the IOL, freeing the IOL from the capsular bag and ensuring the haptics are movable, inserting the new IOL in the bag under the original IOL, and cutting and removing the old IOL.
