Young woman presents with painful ophthalmoplegia in left eye

A 20-year-old woman presented to Tufts New England Eye Center with 1 day of progressively worsening left eyelid swelling and pain with eye movement. Symptoms initially began 10 days before presentation with purulent nasal discharge, which was diagnosed by an outside hospital as viral sinusitis and treated with a short course of oral methylprednisolone and fluticasone nasal spray.
