Pinhole pupilloplasty is a modality to create a pinhole pupil of desired size and location in reference to the vertex of the cornea.
Studies have documented the optimization of pinhole size and its customization according to the patient’s response to a pinhole device placed in front of the patient’s eye with varying diameters ranging from 0.5 mm to 4 mm (gradually increasing in 0.5 mm increments). The pinhole size that allows the best visual acuity is chosen, and an attempt is made intraoperatively to replicate the same.
